Access Expert Care Through the Chronic Disease Management Scheme

If you have had a medical condition or injury for longer than 6 months, you may be eligible for a Medicare referral under the Chronic Disease Management (CDM) scheme. Under the CDM Medicare Scheme, you are entitled to 5 bulk-billed visits to an allied health professional per calendar year.

If you believe you are eligible for a CDM, speak to your GP. They will provide a referral, along with your care plan. This will be sent directly through to us and we will give you a call to book in your first appointment. Alternatively, if your doctor gives a copy of your care plan to you, you can give us a call on 07 5452 5727 to book in your appointment.
